By completing registration for Forum, I hereby consent to and authorize Teladoc Health (“Company”), or anyone authorized by the Company, the perpetual use and reproduction of my name, image, voice and likeness  in video or still, (“Images”) obtained during my attendance at Forum which may be used in connection with the Company’s promotions of its products and services, education and informational campaigns, within an unrestricted geographic area, without further compensation to me.

 

I expressly release the Company, its agents, employees, licensees and assigns from any and all claims that I have or may have for invasion of privacy, defamation or any other cause of action arising out of production, distribution, broadcast or exhibition of the Images.

 

I further understand that the Images may be edited, copied, exhibited, published, or distributed, at the Company’s sole discretion, and hereby waive my right to inspect or approve the finished product, including written copy, wherein my likeness appears. The Images thereof shall constitute the sole property of the Company.

 

You may withdraw your consent any time prior to the end of Forum, on July 21, 2023, by emailing [email protected].
